Why are my labs normal but I feel terrible?
Standard reference ranges are built from the general population, not from healthy people, so they are wide enough to call a decade of decline "normal". A typical physical also skips the markers that explain fatigue — fasting insulin, hs-CRP, homocysteine, and a full thyroid panel are rarely run.
Where "normal" comes from
A lab's reference range is not a definition of health. For most markers it is built by measuring a large sample of people who happened to get that test, then drawing lines around the middle 95% of the results.
That has two consequences worth understanding.
The sample includes people who are unwell. Anyone getting a metabolic panel is, by definition, in a healthcare setting. As population health declines, the range drifts with it — so "normal" is partly a description of everyone else, not a target.
And 95% is a wide net. Being inside it means you are not a statistical outlier. It does not mean the value is where you function best, and it does not mean the value has not moved a long way from where yours used to sit.
The difference between in-range and optimal
Two people can both be flagged normal and be in completely different situations.
Take TSH, with a typical range of roughly 0.4 to 4.5. Someone at 1.2 and someone at 4.2 both get "normal" on the report. They frequently do not feel the same, and the person at 4.2 will often report fatigue, cold intolerance, and slower thinking.
The same applies to ferritin, vitamin D, fasting glucose, and testosterone. This is also why a single result in isolation is less useful than your own trend — a value that has moved from 1.2 to 4.2 over five years is a change, even though every individual report said normal.
The markers a standard physical usually skips
More often than the range being too wide, the explanatory test simply was not ordered. A routine physical typically runs a CBC, a basic metabolic panel, a lipid panel, and sometimes TSH. That misses:
- Fasting insulin. Insulin resistance shows up here years before glucose or A1c move. Rarely ordered.
- hs-CRP. A general marker of inflammation.
- Homocysteine. Related to B-vitamin status and cardiovascular risk.
- Full thyroid panel. TSH alone does not show free T3, free T4, or thyroid antibodies — you can have a normal TSH and meaningful thyroid dysfunction.
- Ferritin. Iron stores drop well before haemoglobin does, and low ferritin causes fatigue with a completely normal CBC.
- Vitamin D and B12. Common, correctable, and strongly tied to energy and cognition.
- Free testosterone and SHBG. Total testosterone alone can be misleading.
What to do with this
Ask for a copy of your actual results with the numbers, not "everything came back fine". Look at where each value sits inside its range rather than just whether it is inside. Compare against your own older results if you have them. And if the panel did not include the markers above, the honest answer is that the test that would explain how you feel has not been run yet.
None of this means every symptom is a lab finding. Sleep, stress, training load, and mental health explain a great deal, and a normal panel with a real symptom is a reason to keep looking rather than to stop.
